Pursuant to §37.2-405, the Office of Licensing licenses services providing treatment, training, support and habilitation: to individuals who have mental illness, intellectual disabilities or substance abuse disorders; to individuals receiving services under the Individual and Family Developmental Disabilities Support Waiver; or to individuals receiving services in residential facilities for individuals with brain injuries. Licensing staff make at least one unannounced inspection of services per year and investigate complaints about licensed services.
Two distinct licenses are available through this office: Children's Residential Services, or All Other Services–Except Children's Residential Services. Here providers can learn about regulations and how to apply for, modify or renew a license.
Use the PROVIDER INSPECTION/INVESTIGATION REPORTS SEARCH to find information about providers in Virginia who are licensed by the DBHDS. Click on the Provider drop-down box (gray arrow), scroll down and click on the desired provider name. Next, click on the Services drop-down box (blue arrow), scroll down and click on the desired provider service-location. Finally, select a Report Type to view any inspection or investigation report completed after January 1, 2012.
Use the LICENSED PROVIDER LOCATION SEARCH to find more information about providers that have been licensed or are in the process of being licensed by the DBHDS. Click on the drop-down box arrows to refine your selections. Click on the "+" (plus sign) to display more detailed information.
Use the DBHDS Intellectual Disability Licensed Providers with Conditional Licenses to view providers and services that have been licensed for the first time. A conditional license is issued to a new provider for services that demonstrates compliance with administrative and policy regulations but has not demonstrated compliance with all the regulations. This listing will be updated on a monthly basis (May 2013).
Use the DBHDS Intellectual Disability Licensed Providers with Provisional Licenses to view providers and services that have a provisional license. A provisional license may be issued to a provider for a service that has demonstrated an inability to maintain compliance with regulations, has violations of human rights or licensing regulations that pose a threat to the health or safety of individuals being served, has multiple violations of human rights or licensing regulations, or has failed to comply with a previous corrective action plan. This listing will be updated on a monthly basis (May 2013).
Use the DBHDS ID Group Homes with four or less beds to view providers that are licensed to group homes for up to four individuals. A Group home residential service" means a congregate service providing 24-hour supervision in a community-based home. Services include supervision, supports, counseling, and training in activities of daily living for individuals whose individualized services plan identifies the need for the specific types of services available in this setting. This listing will be updated on a monthly basis (May 2013).
Use the DBHDS ID Group Homes with five or more beds to view providers that are licensed to group homes for five or more individuals. A Group home residential service" means a congregate service providing 24-hour supervision in a community-based home. Services include supervision, supports, counseling, and training in activities of daily living for individuals whose individualized services plan identifies the need for the specific types of services available in this setting. This listing will be updated on a monthly basis (May 2013).
Use the DBHDS ICF/IID to view providers that are licensed as Community Intermediate Care Faculty/ Intellectual Disability. An Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID)" means a residential facility in which care is provided to individuals who have intellectual disability or a developmental disability who need more intensive training and supervision than may be available in an assisted living facility or group home. This listing will be updated on a monthly basis (April 2013).
