Office of Licensing

Pursuant to §37.2-405, the Office of Licensing licenses services providing treatment, training, support and habilitation: to individuals who have mental illness, intellectual disabilities or substance abuse disorders; to individuals receiving services under the Individual and Family Developmental Disabilities Support Waiver; or to individuals receiving services in residential facilities for individuals with brain injuries.  Licensing staff make at least one unannounced inspection of services per year and investigate complaints about licensed services.

Two distinct licenses are available through this office: Children's Residential Services, or All Other Services–Except Children's Residential Services. Here providers can learn about regulations and how to apply for, modify or renew a license. 

Licensed Provider Search

Use the PROVIDER INSPECTION/INVESTIGATION REPORTS SEARCH to find information about providers in Virginia who are licensed by the DBHDS. Click on the Provider drop-down box (gray arrow), scroll down and click on the desired provider name.  Next, click on the Services drop-down box (blue arrow), scroll down and click on the desired provider service-location. Finally, select a Report Type to view any inspection or investigation report completed after January 1, 2012.

Use the LICENSED PROVIDER LOCATION SEARCH to find more information about providers that have been licensed or are in the process of being licensed by the DBHDS. Click on the drop-down box arrows to refine your selections.  Click on the "+" (plus sign) to display more detailed information.
