Recovery Oriented Systems Indicators Survey
The Recovery Oriented Systems Indicators (ROSI) Survey was designed with and for adults with serious mental illnesses. It measures the kinds of things that help promote recovery (as well as things that don't help recovery) from mental illness. The survey is anonymous. We do not ask for your name or other information that will identify who you are. You may choose not to answer any question or you can stop the survey at any time. The way you answer survey questions will not affect your services. The only risk to you is that sometimes people find that thinking about these kinds of questions can be upsetting. We would like you to complete the survey because the answers you give will help us plan to improve mental health services in Virginia.
(If you agree to take the survey, please read the questions carefully. A few of them are worded in the negative, like "I do not have enough good service options...". "Agree" means that you don't have good service options. "Disagree" means that you do have good service options.)
