Portsmouth Behavioral Healthcare Services - Case Manager (Intellectual Disability)

Job Description: Incumbent will provide case management, service coordination, screening and assessments for consumers with intellectual disability. Work involves identifying individual needs, coordinating resources and monitoring service delivery. Will serve as a primary case manager, provide supportive counseling and intervention, document progress and maintain records in accordance with program policies and procedures. EDUC/EXP: Bachelor’s degree in social work, psychology, education or a related human services field; 1-3 years related experience; or any equivalent combination of training and experience which provides the required knowledge, skills and abilities. Must possess a valid Virginia Driver’s license and be a Qualified Mental Retardation Professional (QMRP). Part C experience is preferred. Qualified applicants may be given a conditional employment offer. Actual employment is contingent upon the satisfactory results of a federal and state criminal background check, using fingerprints and a search of the Virginia Dept. of Social Services’ child abuse and neglect registry, as required by law.

Salary: $31,411
