Middle Peninsula-Northern Neck - Intensive Care Coordinator

Job Description: Under general supervision, employee is responsible for providing intensive care management services to clients identified by local FAP and CPM teams as being at risk of, or currently in, residential treatment facilities Intensive care coordination services providers will be trained in the Virginia High Fidelity Wraparound Model of Care. This will include, coordinating service delivery with providers and community resources, liaisons with court, social and welfare systems, foster ongoing relationships with youth and family members, monitor progress and perform on-site service visits. ICC will report progress regularly with local FAP and CPM teams and make recommendations as appropriate. ICC will participate in weekly supervision by a licensed therapist. REQUIRED: BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and 1-2 years experience desired working with children and adolescents, emergency/crisis services family counseling, family preservation or case management; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities as outlined in the guidelines set forth by the State Executive Council. Salary Commensurate with Education and Experience. Coordinating service delivery with providers and community resources, liaisons with court, social and welfare systems, fostering ongoing relationships with youth and family members, monitoring progress and performing on-site service visits. ICC will report progress regularly with local FAP and CPM teams and make recommendations as appropriate. ICC will participate in weekly supervision by a licensed therapist. The employee will organize and mobilize community resources to meet the needs of children and adolescents who are in residential care. ICC will facilitate a transition back to the community at the earliest and safest appropriate time. Employee provides crisis stabilization/intervention services as indicated by need in population served. Reports to the Mental Health Therapist/Clinical Supervisor. MINIMUM TRAINING AND EXPERIENCE: BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and 1-2 years experience desired in working with children and adolescents, emergency/crisis services family counseling, family preservation or case management; or any equivalent combination of training and experience which provides the required knowledge skills, and abilities. Possession of the required knowledge, skills, and abilities as outlined in the guidelines set forth by the State Executive Council.

Salary: $30, 000-$35,000/Annually
