Middle Peninsula-Northern Neck - Geriatric Specialist
Job Description: Under general supervision acts as liaison between the CSB, nursing homes, assisted living facilities and the community at large as an advocate for the geriatric population. Provides assessment of individual needs and matches them with community resources. Organizes and mobilizes community resources to meet individual needs. Monitors individuals and consults on issues. Conducts individual assessments for admission or recertification to geriatric facilities. Maintains clinical charts and performs other routine administrative functions. Employee must exercise tact and courtesy in contact with staff, individuals, families, public, and services providers. Reports to the Adult MH Care Coordinator. Master’s Degree or witin 6 months of completing in Human Services or related field plus 3-5 years of experience with the knowledge, skills and abilities. State Certified Prescreening Evaluator (SCPE)
Salary: $35,000-$37,500
