Highlands - ID Support Coordinator
Job Description: Intellectual Disability Support Services - The Campus-Annex, Abingdon VA Provides pre-admission/pre-discharge planning for individuals with an intellectual disability who are entering/leaving a state facility, monitors the quality of the individual’s care, provides behavior management training/intervention strategies to families and others who are assisting with supports, completes case documentation of support coordination activities, and maintains necessary casework records to document the provision of support coordination services for Medicaid reimbursement, completes home visits, provides service coordination with other community organizations, assists contractual agencies in developing support plans, monitors compliance with medication prescriptions, providing progress and any side effects to nurse(s) and physician(s), provides consultation and educational activities to individuals and organizations who support individuals in an intellectual disability, provides transportation, consults with supervisor regarding support plans, program status and program coordination, provides consultation (on-call basis) as emergencies arise during off-duty hours, participates in HCS’ Quality Assurance Plan including the peer review of clinical records and satisfaction surveys, represents HCS in meetings, conferences, etc. that have an impact on the provision of support coordination services, and participates in staff development/in-service training programs which have an impact on HCS’ provision of support coordination services. POSITION REQUIREMENTS/QUALIFICATIONS: Bachelor’s degree in human services field and clinical experience required.
