Western Tidewater - CASE MANAGER

Job Description: GENERAL STATEMENT OF RESPONSIBILITIES: Responsible for consumer assessments, service linkage and coordination of services. Assist with development and implementation of Individual Service Plans or Individual Family Service Plans. Provide follow-up counseling, education and instruction to consumers/families. Monitor service delivery with consumers, family members and service providers, and conduct periodic site/home visits. Maintain thorough and timely medical records documentation in accordance with agency, Medicaid and DBHDS standards and requirements. REQUIRED QUALIFICATIONS: BA/BS Degree in Psychology, Social Work, Counseling, Special Education, or related Human Services field; one (1) year of documented experience working directly with individuals who have an intellectual disability; QIDP designation; valid Driver’s License with acceptable driving record at time of employment and continuously thereafter.

Salary: Full Time
