District 19 - ECC Case Manager (2 positions)

Job Description: District 19 Community Services Board, located in historic Petersburg, VA is seeking a highly motivated self-starter to become a full-time Enhanced Care Case Manager for a new Enhanced Care Coordination effort. Successful incumbent must have proven organizational along with excellent verbal and written communication skills. Minimum required qualifications for this position include: Bachelorís degree in a Human Services field or Nursing. Incumbent must meet the qualifications for certification as a QMHP. Experience working with SMI, SA and ID population, proficient computer skills, experience in record keeping/report completion and proficient scheduling/time management skills. Preferred candidates will display knowledge of integration of primary care, basic knowledge of physical health diagnoses and treatments, ability to travel into the community and work with individuals within their home and community setting. Masterís degree in a Human Services field or Nursing degree preferred. Hours are Monday-Friday 8:30 a.m. Ė 4:30 p.m. Requires flexibility based on programmatic need. Starting salary is commensurate with experience and training. Excellent benefits include sick and annual leave, and Virginia Retirement System and group health insurance options. Position above requires a valid VA Driverís License with a point balance of -6 or higher, a pre-employment extensive background check and drug screening.
