Chesapeake - Clinical Case Manager
Job Description: Crossroads Community Services Board has two full time positions to provide case management services to individuals with substance abuse and or mental illness(es) including clinical assessment, supportive counseling, treatment planning, discharge planning, linking with community services, aggressive outreach and follow-up, acting as liaison to public and private treatment facilities in outpatient mental health clinic. Incumbent must meet Medicaid standard for qualified case manager for mental health case management services and possess a bachelor’s degree in human services field with previous work experience in similar setting. Prefer Master’s Degree, meets qualifications as a Certified Pre-screener. Must have valid Virginia drivers license and acceptable driving record. Work Location: Amelia Clinic, Amelia CH, Virginia.
Salary: $32,167 - $36,609 to start
