Chesapeake - CASE MANAGER, Mental Retardation Services

Job Description: Provide case management services to consumers in our service area (counties of Amelia, Buckingham, Charlotte, Cumberland, Lunenburg, Nottoway & Prince Edward). Duties include assisting consumers with accessing community services, coordinating services, developing service plans and keeping accurate records of services provided. Experience with MR Community Waiver preferred. Incumbent must meet Medicaid standard for qualified case manager for mental retardation case management services and possess a bachelor’s degree in human services field with previous work experience in similar setting. Position is located in Farmville, Virginia. Beginning salary: $32,576 with excellent fringe benefits (salary depends upon experience).
