Norfolk - Case Manager III, Intensive

Job Description: Responsible for ensuring that assigned clients with acute symptoms of serious mental illness, who may also have a co-occurring substance use disorder, receive appropriate services throughout the continuum of the mental health and substance abuse service system. The Case Manager will work with clients to identify and access community resources and agencies that can help them achieve their treatment and recovery goals. Qualifications: Bachelor's degree in a human services field and progressive mental health experience to include work with adults diagnosed with serious mental illness and substance abuse disorders and experience in case management. Requires an acceptable driving record.

Salary: $37,794
