Norfolk - Case Manager III, Co-Occurring, Housing First Project

Job Description: Responsible for ensuring chronically homeless clients with mental health and substance abuse diagnoses receive appropriate services throughout the continuum of the mental health and substance abuse service system. Additionally, the Case Manager will work with community resources and referral agencies to identify and activate support services for clients. Qualifications: Bachelor's degree in a human services field and considerable related mental health experience to include work with homeless adults diagnosed with serious mental illness and substance abuse co-occurring disorders. Acceptable driving record and computer skills required.

Salary: $37,794
