Norfolk - Case Manager II, Mental Retardation Case Management

Job Description: Performs all duties necessary to ensure identification and outreach to potential consumers. Also provides continuity of care for consumers with mental retardation by assessing, planning, linking and monitoring. Qualifications: Bachelor's degree in human services field and considerable related mental retardation experience in the case management field. Requires acceptable driving record and computer skills.

Salary: $32,134
